Tourette’s syndrome is a neurological disorder in which there are periodic tics in the muscles. Tics mean sudden contractions. Due to this, there is sudden muscle contraction, and the gestures and words of the victim change.

This syndrome is caused by a disturbance in the balance of neurotransmitters present in the brain.

The symptoms of Tourette’s syndrome are repeated body-mind, some kind of uncontrolled muscle activity, or making any kind of sound from the mouth, throat, and nose.

Most people are unaware of this because its symptoms appear as a habit of the person concerned. 

This syndrome occurs in children from 2 to 14 years of age, which lasts the whole life.

Tics disorders are the persistent tics presence, which are abrupt, repetitive, involuntary movements, and purposeless sounds.

Tics are sudden non-rhythmic behaviors that are either motor (related to movements) or vocal, for example, throat clearing, knee bends, lip-smacking, tongue thrusting, grimacing, eye blinking, and so on.T

Types of tics

Simple Tics

Using only a few muscles or simple words.

Simple motor tics: In this meaningless movement such as facial grimacing, head jerks, shoulder shrugging.

Simple phonic tics: In thismeaningless sounds or noises like throat clearing, coughing, sniffing, barking, or hissing.

Complex Tics

We are using many muscle groups, full words, and sentences.

Complex motor tics: The child has slow and longer purposeful movements such assustained look, facial gestures, biting, banging, whirling, or twisting around. 

Complex phonic tics: These tics include syllables, words, phrases, and statements like “shut up” or “ yes, you’ve done it,” and the child may have abnormal speech. 24% seen in the school children. It seems 3-4 times more common in males than females.

Causes of tics disorder

  • Considered emotional factors considered a cause of tics, but now the search for reasons focuses on biological, chemical, and environmental factors. 
  • Both functional and structural abnormalities in brains
  • Abnormal neurotransmitters contribute
  • Changes within the brain, especially basal ganglia, and anterior cingulate cortex.

Medications

Medicines are the primary treatment for motor and vocal tics. Medications prescribed include:

Typical neuroleptics (antipsychotics) like Haloperidol, Pimozide, and Tetrabenazine.

Alpha-adrenergic receptor agonist, including Clonidine and Guanfacine.

Atypical Antipsychotics like Risperidone and Clozapine.

Cognitive behavior therapy 

The patient asks to deliberately perform tic movement for a specified period interspersed with brief rest periods.

Contingency management is another behavioral treatment that the parents administer.  Repeated Habit is the most commonly used technique when combined with relaxation exercises and awareness training.

Prevention of tics disorder

  • Maternal emotional stress during pregnancy and severe nausea and vomiting during the first trimester may affect tic severity. Should minimize during pregnancy.
  • Attempting to minimize prenatal stress may serve a limited preventive function. 
  • Tic disorder is sensitive to pressure; maintaining a low-stress environment can reduce the number or severity of tics. 
  • Gives positive reassuring to the child and family.  
  • should do an Assessment done to assess cognitive abilities, perception, behavior, adaptive functioning, and motor skills.
